Your Gut Didn't Suddenly Break

One day, you can enjoy a salad, a cup of coffee, or your favorite restaurant without giving it a second thought. The next, you're wondering why you're bloated by noon, constipated for three days, or running to the bathroom after a meal you've eaten dozens of times before.

Maybe you've noticed that dairy suddenly doesn't agree with you. Or wine gives you a headache. Perhaps foods that once made you feel energized now leave you feeling sluggish and uncomfortable.

Digestive complaints are incredibly common during perimenopause, yet they rarely make the list of symptoms women are told to expect. While hot flashes, night sweats, and irregular periods get most of the attention, many women first notice that something feels different in their digestive system. Emerging research and clinical experience both suggest that hormonal changes during the menopause transition can influence gut function, gut microbes, and the way our digestive system responds to stress.¹

Understanding why these changes happen is the first step toward supporting your body instead of fighting it.

Perimenopause Is About Much More Than Your Ovaries


When we think about perimenopause, we often think about estrogen and for good reason.

Estrogen is one of the primary hormones changing during this transition. But what many women don't realize is that estrogen isn't just involved in reproduction.


Estrogen receptors are found throughout the body, including your brain, bones, cardiovascular system, immune system, skin, and digestive tract.


For years, estrogen has quietly supported many aspects of digestive health by helping to:

  • Maintain the integrity of the intestinal lining

  • Influence gut motility (how food moves through the digestive tract)

  • Support immune function within the gut

  • Interact with the trillions of microbes living in your intestines

  • Help regulate inflammation

  • Participate in the metabolism and elimination of hormones


As estrogen begins to fluctuate during perimenopause, it's normal for these systems to become a little less predictable.


Some women experience constipation because progesterone and changing estrogen levels can slow intestinal movement. Others develop looser stools, increased urgency, or alternating bowel habits. Many notice more bloating or become increasingly sensitive to foods that never used to bother them.


Not every woman experiences every symptom, and no two women experience perimenopause in exactly the same way. One of the reasons is that your gut doesn't exist in isolation. It reflects the story your body has been writing for years.


Meet Your Second Brain


Have you ever been so nervous before giving a presentation that your stomach tied itself in knots? Or lost your appetite after receiving bad news? Maybe you've had "butterflies" before an exciting event. Those experiences remind us that our brain and digestive system are in constant conversation.


Scientists call this relationship the gut-brain axis, a remarkable communication network involving the brain, the vagus nerve, hormones, immune cells, and the gut microbiome.²

Rather than thinking of your digestive system as a long tube that simply processes food, imagine it as an incredibly intelligent communication center.


In fact, your digestive tract contains hundreds of millions of neurons, so many that it's often called our "second brain." Every day, your brain and gut exchange millions of messages.

When everything is working together, digestion feels effortless. Food is broken down efficiently. Nutrients are absorbed. The intestinal barrier remains healthy. Your microbiome thrives. Most of the time, you don't even think about it. Until something changes.


The Gut-Brain-Hormone Connection: Why Stress Suddenly Hits Your Gut Harder


Have you ever noticed that your digestion is better on vacation?

You eat foods you normally avoid, indulge in dessert, maybe even enjoy a glass of wine and somehow your stomach behaves itself.


Then you come home, and the bloating, constipation, or urgent trips to the bathroom return.

Was it the food?


Sometimes.


But often, it's something much deeper.


Your digestive system isn't controlled only by what you eat. It's also controlled by your nervous system.


Think of your brain and gut as constant texting buddies. Every second of every day, they're exchanging messages through hormones, neurotransmitters, immune cells, and the vagus nerve, the primary communication highway between your brain and your digestive tract.


When your brain senses safety, digestion flourishes.


When it senses stress, digestion takes a back seat.


That response made perfect sense thousands of years ago. If our ancestors were running from danger, digesting lunch wasn't the body's top priority. Blood flow shifted toward the muscles, heart rate increased, stress hormones surged, and digestion temporarily slowed.

The challenge is that today's "threats" rarely look like predators.


Instead, they look like:

  • Constant multitasking

  • Caring for aging parents while raising children

  • Work deadlines

  • Financial pressure

  • Poor sleep

  • Overtraining

  • Undereating

  • Living in a constant state of "go"


Your nervous system doesn't necessarily distinguish between a tiger and an overflowing inbox. It simply responds to what it perceives as stress.


During perimenopause, that stress response often becomes even more sensitive.

Research suggests that fluctuating estrogen levels can influence the body's stress response through interactions with the hypothalamic-pituitary-adrenal (HPA) axis, helping explain why many women feel less resilient to stress during this life stage.


Why Digestion Suffers When Your Nervous System Doesn't Feel Safe


When your body spends too much time in a fight-or-flight state, digestion changes in very real ways.


You may produce less stomach acid, making it harder to break down proteins and absorb nutrients like iron, zinc, calcium, and magnesium and B12. Digestive enzyme secretion may decrease, leaving meals feeling heavier than they used to. Gut motility can become erratic. For some women, everything slows down and constipation becomes the new normal. For others, stress speeds everything up, leading to urgency or loose stools. Chronic stress can also influence the intestinal barrier and alter the composition of the gut microbiome, which may contribute to digestive symptoms in susceptible individuals.² This is one of the reasons I spend so much time talking with clients about nervous system regulation.


Perhaps most importantly, chronic stress can reshape the gut microbiome itself. Over time, it may reduce the abundance of beneficial microbes while allowing less favorable organisms to thrive, creating a cycle in which stress affects the gut and the gut, in turn, influences mood, energy, and resilience.


This is why digestive symptoms are never "just in your head." They are real, measurable, physiological responses to the signals your nervous system is sending throughout your body.


Your nervous system helps determine how efficiently every digestive organ does its job.

Healing your gut isn't just about what you eat. It's also about creating an internal environment where your body finally feels safe enough to digest, absorb, and repair.


Your Gut Microbiome Is Changing Too


Inside your digestive tract live trillions of bacteria, fungi, viruses, and other microorganisms collectively known as the gut microbiome.


For years, scientists thought these tiny organisms primarily helped digest food.


Today, we know they do so much more.


A healthy microbiome helps:

  • Digest fiber and produce beneficial compounds like short-chain fatty acids (including butyrate)

  • Support a healthy immune system

  • Strengthen the intestinal barrier

  • Communicate with your brain

  • Influence inflammation

  • Help regulate hormone metabolism


In many ways, your microbiome acts like a bustling ecosystem. Just as a thriving forest depends on biodiversity, your gut depends on a wide variety of beneficial microbes working together.


And like any ecosystem, it's influenced by its environment.

The foods you eat.

Your sleep.

Stress.

Physical activity.

Medications.

Alcohol.

Travel.

Even your hormones.


As estrogen begins to fluctuate during perimenopause, the environment these microbes live in changes too. Research suggests that hormonal shifts can influence the composition and function of the gut microbiome, although the exact changes vary significantly from one woman to another.⁴


That's an important point. There isn't one "perimenopause microbiome." Every woman brings decades of life experiences into this transition. If you've spent years eating a colorful, fiber-rich diet, moving your body, sleeping well, and managing stress, your microbiome may adapt more easily.


If you've experienced chronic stress, repeated antibiotic use, restrictive dieting, digestive infections, or years of blood sugar instability, perimenopause may simply expose vulnerabilities that have been quietly developing for years.


Again...


Your body isn't failing.

It's asking for support.


The Estrobolome: One of the Coolest Things You've Probably Never Heard Of


Now let's introduce one of my favorite topics.

The estrobolome.


It sounds complicated, but the concept is actually pretty simple. Your liver is constantly processing hormones, including estrogen. Once estrogen has done its job, your liver packages it for elimination and sends it into the digestive tract. Ideally, that estrogen leaves your body through the stool. But here's where your gut microbiome enters the story.

Certain bacteria produce an enzyme called beta-glucuronidase.

This enzyme can "unwrap" some of that packaged estrogen before it's eliminated.


Sometimes that's helpful. Sometimes it's not.


Like almost everything in biology, it's all about balance. Too much reactivation may contribute to higher circulating estrogen in some situations. Too little may contribute to lower estrogen availability. Scientists are still learning exactly how the estrobolome influences women's health, but research increasingly suggests that maintaining a healthy, diverse microbiome supports healthy estrogen metabolism.⁵


This is one reason why I rarely jump straight to aggressive gut-cleansing protocols.

Our goal isn't to wipe out bacteria.

Our goal is to cultivate a resilient ecosystem.


Think of yourself as tending a garden rather than declaring war on weeds.

Why Your Favorite Foods Suddenly Don't Love You Back


One of the most frustrating parts of perimenopause is feeling like foods you've eaten for years suddenly betray you. Bread makes you bloated. A glass of wine gives you a headache. A restaurant meal leaves you feeling puffy for two days. Coffee suddenly makes your stomach churn.


It's tempting to assume you've developed dozens of food intolerances overnight.

Usually, that's not what's happening. Instead, several normal physiological changes may be occurring at the same time.


Your digestion may be slowing.

Food sits in the stomach a little longer.

You feel fuller sooner.

Meals feel heavier.


Your gut bacteria may be shifting.

Certain microbes flourish while others decline, changing how foods are fermented and influencing gas production.


Your nervous system may be more reactive.

When your body perceives stress, digestion becomes less coordinated, making even healthy meals feel uncomfortable.


Your immune system may become more sensitive.

If the intestinal barrier has become more permeable because of chronic stress, inflammation, medications, or illness, your immune system may respond differently to foods than it once did.


Your hormone fluctuations may amplify everything.

The same meal can feel completely different depending on where you are in your hormonal cycle.


One day you tolerate it beautifully.

The next day you're bloated before you've finished eating.

That's one reason food journals can sometimes feel confusing.

It's rarely just the food.

It's the context in which your body receives that food.


What About Histamine?


This is one area I see becoming increasingly common in my practice.

Some women notice that during perimenopause they become more sensitive to foods like:

  • Wine

  • Aged cheeses

  • Fermented foods

  • Leftovers

  • Smoked meats/fish

  • Vinegar

  • Tomatoes


Does that automatically mean you have histamine intolerance? Not necessarily.


Histamine metabolism is influenced by multiple factors, including genetics, gut health, medications, immune function, and hormones.


Interestingly, estrogen and histamine have a two-way relationship.

Estrogen can promote histamine release, while histamine can stimulate additional estrogen activity in certain tissues, creating a feedback loop that may make some women feel more symptomatic during periods of hormonal fluctuation.⁶


That doesn't mean everyone needs a low-histamine diet.

In fact, long-term restrictive diets often reduce dietary diversity, which may work against the very microbiome we're trying to nourish. Instead, I encourage clients to think of food elimination as a temporary investigation, not a lifelong identity.


Our goal is always to expand the diet whenever possible.

Because every colorful plant you tolerate is another opportunity to nourish your microbiome.


Blood Sugar: The Piece Almost Everyone Misses


If you've worked with me before, you probably knew this section was coming.


I truly believe blood sugar is one of the most overlooked contributors to digestive symptoms during perimenopause.


Every cell in your body depends on a steady supply of energy. Your brain especially. When blood sugar drops too low, your body doesn't shrug its shoulders and hope for the best. It responds immediately. Adrenaline rises. Cortisol rises. Stored glucose is released into the bloodstream.


It's an elegant survival system.


But when this happens several times every day, because of skipped meals, under-eating, relying on caffeine, or meals that lack enough protein and fiber, it sends a different message to your nervous system.


Resources are scarce.


That message makes it much harder for your body to settle into the "rest and digest" state your gut depends on.


This is one reason I spend so much time helping clients build meals that include:

  • High-quality protein

  • Healthy fats

  • Fiber-rich carbohydrates

  • Plenty of colorful plants


These foods don't just stabilize blood sugar. They help create an internal environment where your digestive system can do what it was designed to do.


I often tell clients:

Stable blood sugar is one of the first safety signals your nervous system receives.

And a calmer nervous system is often one of the best things you can do for your gut.


Five Ways to Support Your Gut During Perimenopause


If there's one thing I hope you take away from this article, it's this:

Your body isn't asking for perfection. It's asking for support and consistency.


Perimenopause isn't a problem to fix. It's a transition to navigate. While you can't stop your hormones from changing, you can create an environment that helps your body adapt more gracefully.


Here are the five foundations I focus on with nearly every client.


1. Feed Your Microbiome Every Day

One of the best ways to support your gut is NOT found in a supplement bottle.


It's on your plate.


The beneficial bacteria in your colon thrive on fiber particularly the kinds found in vegetables, fruits, legumes, nuts, seeds, herbs, spices, and whole grains (when tolerated). As they ferment these fibers, they produce compounds called short-chain fatty acids, including butyrate, which helps nourish the cells lining the colon and supports a healthy intestinal barrier.⁷


Rather than chasing the newest superfood, focus on variety. Aim to "eat the rainbow" throughout the week. Different plant colors provide different phytonutrients, and different fibers feed different microbes. Think of every colorful meal as planting seeds in your internal garden. You don't have to be perfect. You just have to keep tending the garden.

Beth's Clinical Pearl I encourage clients to think about adding foods before eliminating them. More colorful vegetables. More herbs. More berries. More beans if tolerated. More seeds. More diversity. Your microbiome loves variety.

2. Build Meals That Keep Blood Sugar Steady

I know you've heard me say it before.

That's because it matters.

Every balanced meal is another opportunity to tell your nervous system:

"You are safe."

One of the simplest ways to do that is by including:

  • Approximately 30+ grams of protein

  • Healthy fats

  • Fiber-rich carbohydrates

  • Plenty of colorful vegetables


This combination slows digestion, supports more stable blood sugar, and provides a steady supply of energy throughout the day. It also helps reduce the dramatic blood sugar swings that can trigger cortisol release and leave you feeling wired, hungry, and exhausted all at once.


Your digestive system thrives on consistency. Your nervous system does too.


3. Support Your Nervous System as Intentionally as Your Nutrition


This is the piece I believe is often missing from conversations about gut health.


You can eat the most nutrient-dense diet in the world. But if your body spends every day believing it's under attack, digestion will always be working uphill. Supporting your nervous system doesn't require hours of meditation. Sometimes it's the small things done consistently that make the biggest difference.


  • A few minutes of slow breathing before meals.

  • Eating without scrolling your phone.

  • Walking outside after dinner.

  • Strength training a few times each week.

  • Laughing with friends.

  • Sleeping enough.

  • Spending time in nature.

Creating moments throughout the day when your body receives the message:

"You're safe."


Those moments matter more than most people realize.


4. Listen to your Your Gut Instead of Fighting It

One of the biggest mistakes I see is women declaring war on their digestive system.

Every symptom becomes something to suppress.

Every uncomfortable meal becomes another food to eliminate.

Soon they're eating six foods and wondering why they still don't feel well.


Instead, become curious.


Ask questions.

When did this start?

Is it every meal or only certain meals?

Is it worse during stressful weeks?

Does travel change it?

How's my sleep?

How's my blood sugar?

How's my stress level?


Symptoms aren't your enemy. They're information. They're your body's way of asking you to pay attention.


5. Remember That Healing Is an Ecosystem

There is no single food.

No miracle probiotic.

No perfect supplement.

No magic lab test.


Healing happens when enough small things begin moving in the same direction.


When you consistently nourish your microbiome. When you stabilize your blood sugar. When you build muscle. When you sleep. When you move. When you laugh. When you regulate your nervous system. When you choose progress over perfection. Those small decisions begin creating an internal environment where your body can finally do what it has wanted to do all along:

Heal.


The Bottom Line

You're not imagining it. And you're certainly not alone.


The hormonal changes of midlife influence far more than your menstrual cycle. They can affect the gut microbiome, the intestinal barrier, the gut-brain axis, immune function, stress resilience, and the way your body processes food.

Perimenopause often reveals the places where we've been running on empty for years.

Yes. Many women notice increased bloating during perimenopause due to hormonal fluctuations, changes in gut motility, alterations in the gut microbiome, and increased sensitivity to stress. However, persistent or severe bloating should always be discussed with your healthcare provider to rule out other causes.


Can perimenopause cause constipation?


Yes. Fluctuating estrogen and progesterone levels may influence how quickly food moves through the digestive tract. Stress, reduced physical activity, dietary changes, and inadequate fiber or hydration can also contribute.


Can hormones change the gut microbiome?


Research suggests that sex hormones and the gut microbiome influence one another in complex ways. Scientists are still learning exactly how these relationships work, but it's clear that hormonal transitions like perimenopause can affect the gut environment.⁴


Should I take a probiotic?


Maybe, but not automatically.

While probiotics can be helpful for some people, they're only one piece of the puzzle. Nutrition, sleep, stress management, blood sugar regulation, movement, and overall dietary diversity often have an even greater impact on long-term gut health. I rarely give a probiotic in my first session with a new client. Food and lifestyle first, always.
